MADISON — Kindergarten registration and screening for students who reside in Madison will be Wednesday, April 15, and Thursday, April 16, at Madison Elementary School.

A child must be 5 years old on or before Oct. 15. To schedule an appointment, call the elementary school at 696-4607, or stop by the office. Children will be registered and screened by appointment only. Appointments are being accepted now.

Parents/guardians are asked to bring the child, his/her birth certificate, current immunization records and a copy of a current 4- or 5-year-old physical to the registration appointment.

The school board of Maine School Administrative District 59 has adopted a policy that requires all children be up to date with immunizations including the varicella vaccine (chicken pox) and physical exams, according to a news release from the school. These up-to-date records must be on file with the school nurse by the first day of the 2015-16 school year.

A family handbook containing information regarding kindergarten screening and programming is available in the elementary school office. All are encouraged to pick one up prior to the child’s registration appointment.
